The BM Premier Trust SIPP

This page contains the literature and forms needed to open a BM Premier Trust SIPP. The BM Premier Trust is a SIPP that we have set up with Brooks MacDonald Asset Management. All assets are investd with Brooks MacDonald Managed Portfolio Service and all costs are paid fom an annual management charge.

Before you decide to proceed with the SIPP it is essential that you download, read and understand the Key Features document and Charges sheet. These give you basic information about the product.

A SIPP is not suitable for everyone and we recommend that you speak to an independent financial adviser before you decide whether to proceed.

If you would like to proceed with opening a SIPP then you will need to provide us with the following:

1. Completed and signed Application Form (available below).

2. Completed and signed Member Bank Account Mandate (available below).

3. Proof of your identity, in the form of:

a) Original documents such as your passport or photo driving licence and a utility bill dated in the last three months, or

b) An Introduction Certificate which must be completed by your Financial Adviser (available below).
